<p><p>The Terraces of Phoenix, a premier 5-Star HumanGood life plan community, is seeking a compassionate PRN Licensed Practical Nurse (LPN/LVN) for our Skilled Nursing Team.</p> <p><strong>** PRN/Per Diem ** Must have 3-11 PM and 11 PM-7 AM Shift Availability </strong><br /><strong>** Pay rate: $38.00 - $45.00 (DOE)</strong></p> <p>Our Licensed Practical Nurses (LPNs) are responsible for the nursing needs of residents, while actively incorporating our “Philosophy for Person-Directed Care.” You will ensure a safe and healthy environment for residents, staff, and visitors to participate in the resident’s daily life rhythm; develop assignments in coordination with the care partners; and keeping each team member accountable for resident care and satisfaction.</p> <p> </p> <p><strong>What you will need to be successful in these roles:</strong></p> <ul> <li>Current LPN license in the state of Arizona</li> <li>Current CPR Certification</li> <li>Experience in skilled nursing, subacute care, admissions, or case management is highly desirable.</li> <li>Excellent assessment, documentation, communication, and organizational skills.</li> <li>Strong customer service and teamwork acumen.</li> <li>Current or prior experience with PCC is preferred.</li> </ul> <p> </p> <p><strong>What's in it for you? </strong></p> <p>At HumanGood, we’re driven by purpose—reimagining senior living and creating communities where older adults can thrive.
