<div data-field="description" class="jd-description">Key responsibilities:<ul><li class="x">Triaging and managing patient messaging and communication via phone and patient portal </li><li class="x">Assessing, planning, implementing, evaluating and modifying plans of care for a specific patient assignment throughout the healthcare continuum.</li><li class="x">Providing exemplary patient care and working closely with physician partners and members of the interdisciplinary Epilepsy team.</li><li class="x">Contacting patients for changes in clinical management.</li><li class="x">Providing ongoing care coordination between clinic visits by communicating with families, healthcare team members and other agencies involved.</li><li class="x">Educating patients and families regarding disease management and medications.</li><li class="x">Assisting with completion of prescriptions, school forms, medication and DME insurance authorizations, and other community networking resources and referrals</li><li class="x">Working 40 hours per week: (4) 10-hour shifts Monday through Friday </li></ul>Minimum qualifications: Education:<ul><li class="x">BSN degree</li></ul>Experience:<ul><li class="x">At least two years of Pediatric RN experience in either inpatient or ambulatory setting required</li><li class="x">Neurology and/or epilepsy experience preferred</li></ul>License/certification:<ul><li class="x">MA RN license</li></ul> </div><div data-field="payStatement" class="jd-payStatement">The posted pay range is Boston Children’s reasonable and good-faith expectation for this pay at the time of posting.Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors.
