This position also serves as a mentor to new associates in coordination with the orientation plan of the community.</span></p><p style="margin: 0px;"> </p><p style="margin: 0px;"><strong><span style="font-family: verdana, geneva; font-size: 12pt;"><u>Essential Functions, Key Duties, & Responsibilities</u></span></strong></p><ul><li><span style="font-family: verdana, geneva; font-size: 12pt;">Serve as a mentor to new associates.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Maintain structured mentoring relationship through first year of employment using a positive approach and focusing on strengths while promoting independence and problem </span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Monitor progress of new associates, completing mandatory competencies and validating that competencies are completed. </span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Commit time and share expertise to help guide new associates to learn desired skills.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Meet at regular intervals (as designated by department guidelines) to provide feedback and coaching, evaluate progress, and help resolve </span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Help new associates to feel a sense of belonging in the team and organization by fostering relationships with all team members.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Provide ongoing feedback to supervisor on the status of new associates’ progress.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Provide supportive services to nurses(s) and other associates as needed.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Assist with or perform activities of daily living for residents in accordance with care plans and established community policies and procedures.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Assist residents with lifting, turning, transferring, positioning and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Coordinate dining room services at assigned mealtimes, including set-up, clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Assist with monitoring the condition of residents’ skin, reporting any skin issues (reddened areas, pressure areas, skin tears, etc.) to the nurse and supervisor.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Deliver nutritional supplements to residents at assigned times and provide resident assistance as needed to ensure intake, documenting intake per community policy/procedure.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Participate in life enrichment programming and providing meaningful activities to residents.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Document resident care timely and accurately.</span></li><li style="margin-bottom: 0in;"><span style="font-family: verdana, geneva; font-size: 12pt;">Identify residents’ physical and cognitive changes, making appropriate follow-up decisions, documentation, and communication to appropriate associates.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Assist the residents with leisure time programming, including escorting them to and from activities.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Report any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and Administrator/Executive Director.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Follow appropriate safety policies and procedures at all times for the protection of all residents and associates.</span></li><li><span style="font-family: verdana, geneva; font-size: 12pt;">Adhere to all infection control policies and procedures.</span></li><li style="margin-bottom: 0in;"><span style="font-family: verdana, geneva;
