Family Practice/Primary Care Nurse Practitioner

<p>CenterWell is looking for a dedicated Advanced Practice Provider who is Board Certified/Board Eligible for an exciting primary care role.<br>At CenterWell, you'll be more than a healthcare provider you ll be an integral part of each patient s journey. Our unique care model places a keen focus on senior primary care with a harmonious multidisciplinary approach.<br>A Day with CenterWell:<br>See 12-15 patients per day, allowing for personalized patient interactions<br>Play a crucial role in a value-based care model, boosting patient's quality of life<br>Handle a diverse caseload from preventive care to chronic conditions. <br>Dedicate 100% of your time to direct patient care, including patient-facing time and associated administrative tasks<br>Active, unrestricted NP/PA license in the state<br>Current NP or PA certification<br>Current DEA license or the ability to obtain one<br>Passion for enhancing consumer experiences within an organization<br>This role is patient-facing and part of Humana/Senior Bridge's TB screening program.
