Call or text at , or send your updated resume to for immediate consideration. </p> <p> Assignment Details </p><ul><li> Start Date: November 1, 2025 </li><li> Duration: 6 months with perm conversion offered </li><li> Schedule: Mon Fri 8:00 AM 5:00 PM </li><li> Pay: Call for rates + Benefits (Health, Dental +Vision) + 100% Malpractice covered. </li> </ul> <p> Clinical Scope </p><ul><li> Primary care for geriatric patients with polychronic conditions </li><li> In-home visits across Los Angeles County </li><li> Patient Volume: 3 7/day </li><li> EMR: Athena </li><li> Support Team: 1 APP + 1 MA </li> </ul> <p> Requirements </p><ul><li> Board Certified NP </li><li> Active California NP license </li><li> Certifications: BLS, DEA </li><li> Experience: </li><ul><li> Minimum 2 years of direct patient care </li><li> Must have in-home care and geriatric experience </li><li> Nephrology experience is a plus! </li> </ul> </ul> <p> Must live within 20 miles of central Los Angeles Travel radius: Up to 1 hour Mileage reimbursed at $0.70/mile </p>
