They currently boast a team of more than 25 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider s office. Their services include, among others, primary care, psychiatric care, post-acute care, and transitional care.<br>POSITION DESCRIPTION: <br>The nurse practitioner will serve as the primary care provider for a dedicated panel of geriatric patients located within a defined geographic territory within Cincinnati. Patients are located in private homes (house calls), as well as long-term care facilities (ALFs, ILFs).<br>Providers see a variable number of patients per day, dependent upon type of visit, travel time, and patient needs. <br>Dedicated schedulers put together daily schedules and routes for the providers to ensure efficiency and appropriate time for every visit.<br>Specific duties of the role include, but are not limited to:<br>- conduct health assessments to identify patient needs<br>- develop personalized care plans<br>- regularly evaluate patient conditions and responses to treatment<br>- adjust treatment plans as necessary<br>- perform minor procedures and wound care as required (and as settings permit)<br>- educate patients and families on health management, disease prevention, etc.<br>- provide guidance on lifestyle modifications and self-care practices<br>- work with physicians, nurses, and other professionals to coordinate care<br>The practice uses a mobile-based EHR with AI functionality to reduce administrative time.<br>On-call is phone triage only with very low volume.
