This role ensures compliance with regulatory standards and promotes continuous improvement in patient care and organizational performance.</span></p><p> </p><p><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;"><strong>Primary Responsibilities:</strong></span></p><ul><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Supervise and evaluate QAPI personnel, including performance management and development</span></li><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Oversee the monitoring, implementation, and evaluation of programs to ensure compliance with regulatory, accreditation, and professional standards</span></li><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Lead the agency's Quality Assurance and Performance Improvement (QAPI) program, ensuring processes are in place to measure and improve performance</span></li><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Direct and coordinate quality initiatives using continuous quality improvement (CQI) methodologies</span></li><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Develop improvement models and provide education/training to staff on performance improvement and team development in collaboration with key stakeholders</span></li><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Ensure effective monitoring and evaluation of safety, risk management, and infection control programs</span></li><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Collaborate with clinical leadership to support staff orientation, competency validation, and ongoing education programs that meet regulatory requirements</span></li><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Ensure all QAPI activities are documented, tracked, and reported appropriately</span></li><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Schedule and facilitate monthly QAPI meetings, ensuring accurate documentation and storage of meeting minutes (e.g., SharePoint)</span></li><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Oversee preparation of reports and presentations related to quality and performance metrics</span></li><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Identify staff education and training needs based on data trends and performance outcomes</span></li><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Monitor high-risk indicators, analyze root causes, and implement corrective Performance Improvement Plans (PIPs)</span></li><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Coordinate the review of complaints, incidents, appeals, and regulatory survey outcomes with home health staff</span></li><li><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">Provide input into employee performance evaluations as it relates to quality and compliance</span></li></ul><p> </p><p><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;"><strong>Additional Information</strong></span></p><p><span style="font-family:Tahoma, Geneva, sans-serif;font-size:10px;">This description is intended to summarize the primary duties and responsibilities of the role.
